Plans, implements, monitors and evaluates care delivery for patients with sepsis while identifying and integrating patients' learning needs, abilities and readiness to learn into plans of care. Utilizes standards, guidelines, pathways and evidence based practice for sepsis identification and care delivery, while incorporating data, analytics and electronic health records to continuously improve care and practice to enhance outcomes. Acts as a consultant and subject matter expert in the clinical setting by maintaining and promoting standards of care and having respect for and an understanding of the contributions of all team members.
